Adults: A Continuum of Care

Adult Outpatient

The overall goal of all Pacific Clinics' adult programs is to assist consumers by establishing and working toward their personal goals to maximize their levels of productivity in the community and to improve their individual quality of life. Pacific Clinics' adult outpatient services include psychosocial rehabilitation, intensive psychiatric evaluation and treatment, case management services, individual and group/family therapy, crisis intervention, vocational counseling, housing services, substance abuse/mental health groups, recreational therapy, clubhouse/drop-in centers, and self-care programs as needed in a coordinated, comprehensive treatment plan with an emphasis on achieving consumer-set goals for recovery.

Assertive Community Treatment (ACT)

Staff is on-call 24/7 for a wide range of consumer needs, providing intensive treatment for those with serious and persistent mental illness. Referrals for services are made based on individual needs. Asian and Latino cultural and language capabilities for monolingual consumers and their families are available.

AB34 Services (also see FSP Services)

Developed from a successful statewide pilot project with Pacific Clinics as one of its three initial providers, this program provides outreach and integrated community-based treatment services to individuals with a serious mental illness who may be homeless or had involvement with the criminal justice system. The program offers mental health services including case management, medication support, 24/7 crisis intervention per week, housing and vocational assistance, assistance with access to healthcare, consumer self-help and peer support, treatment for co-occurring substance abuse and money management.

CalWORKs

The program, working by referral from the Department of Public Social Services, assists and supports individuals, most with children, whose mental health issues make it difficult for them to secure steady employment and become self-sufficient, independent of the state welfare system. Services provided focus on mental health and vocational counseling, educational/training opportunities, work preparation, job placement and, more recently, on housing issues. The program also addresses childcare and other family member needs. Pacific Clinics operates eight sites in two counties.

Unique programs in five Pacific Clinics counties for clients at varying levels of recovery provide comfortable, welcoming, consumer-run settings where members may engage in peer support activities, improve their social interaction, and access educational, vocational and recreational opportunities.

Full Service Partnership Programs

Pacific Clinics is proud to be a Full Service Partnership (FSP) provider serving adults ages 26-59. Funded by the Mental Health Services Act (MHSA), Pacific Clinics multicultural staff reaches out to ensure access to these recovery-based programs for unserved, underserved, and inappropriately served consumers. Also engaging the homeless, recently incarcerated and other high-risk individuals through targeted outreach in what were formerly called AB34/AB2034 programs, Pacific Clinics staff provides intensive field-based services and treatment for co-occurring mental illness and substance abuse to those in greatest need. Other supportive programs include housing and vocational services. Housing

Increasingly Clinics-wide services place recovering consumers in temporary, transitional, and permanent homes. In collaboration with state and federal programs, eligible consumers are placed in subsidized housing with behavioral health and supportive services.

Integrated Treatment /SAMHS

Consumers with co-occurring substance abuse and mental health disorders are more effectively served by utilizing one treatment plan with an interdisciplinary team of professionals, giving both staff and consumer access to a wide range of supportive services as an effective treatment strategy. Programs are available at most Pacific Clinics locations.

Passageways Homeless Multi-Service Center

In this collaborative and mainly HUD-funded project with several local Pasadena agencies, staff members help consumers with securing critical benefits, money management, temporary and permanent housing, health and drug issues, and other supportive services. Similar services are available at the Whittier/Santa Fe Springs One-Stop Center.

Wraparound Assisted Treatment (WAT)

For consumers who have committed a misdemeanor crime because of their mental illness, the WAT program serves stabilized clients voluntarily assigned to the program through the mental health court system for about six months to a year. On completing their court requirement, consumers may be referred to other mental health programs.

Wellness Recovery Action Plan (WRAP)

As part of a self-help program that offers adult and families better ways to care for themselves and their loved ones, development of their own unique WRAP’s commit consumers to learning skills for monitoring and reducing symptoms, and options for improving their quality of life. Currently available at thirteen of the Clinics' adult services or family programs, it is also being introduced into young adult programs.

Vocational Services

Important to consumers who want to achieve the ability to work, the El Monte-based program offers extensive services, especially to other Pacific Clinics' programs such as CalWORKs. In compliance with a state requirement, the program has been accredited since 1996 by the nationally recognized CARF organization.

Linkage and Referrals

The Pacific Clinics Call Center at

provides a consumer-friendly toll free entry to Pacific Clinics. Intake appointments at various locations may be scheduled for most child, adult and family programs. Call Center staff also help consumers link to other resources available for follow-up in the community where they live.
