On the street and out of options, life for young adults, or what is called Transitional Age Youth, ages 16-25 years old can be cold, dangerous, and a dead end. Pacific Clinics reaches out to homeless youth, particularly those with mental health and substance abuse problems with programs that focus on individual strengths to create pathways to productive lives.
Full Service Partnership Programs
Pacific Clinics is proud to be a Full Service Partnership (FSP) provider serving teens and transitional age youth. Pacific Clinics multicultural staff reaches out to ensure access to these recovery-based programs, funded through the Mental Health Services Act (MHSA), for unserved, underserved, and inappropriately served consumers. Through outreach and engagement to the homeless, recently emancipated and incarcerated, and other high-risk youth, Pacific Clinics staff provides intensive field-based services and co-occurring treatment to those in greatest need. Other supportive services include housing and vocational services. With FSP funding, these targeted services for young people are provided in a welcoming setting at Wellness or Drop-In Centers in many of the counties with Pacific Clinics.

Healthy Transitions
The program that initiated Pacific Clinics’ now-expanded TAY services in the San Gabriel Valley, Healthy Transitions engages homeless transitional age youth (TAY) on the streets, parks, and other meeting places. Healthy Transitions Staff provides food, blankets, emergency housing, referrals, and "whatever it takes" to assist young consumers through crisis. Then, as they commit to participating in an assessment and treatment plan or intensive case management services, staff helps them identify their goals and supports their achievement of them at the clients' own pace. Transitional age youth are also provided services at programs in Glendale, Irwindale, Oxnard, Pasadena, Rancho Cucamonga, Santa Fe Springs, and Yucca Valley/Morongo Basin.
Hestia House: A Safe Place to Call Home
Named after the goddess of the hearth, Hestia House in Pasadena is a sheltering place from the dangers of life on the streets. The residents participate in counseling, treatment, and education to overcome addiction and mental illness in a new setting where they learn to take responsibility for shaping their futures.
