Integrated Behavioral Health Care
Our Certified Community Behavioral Health Clinic (CCBHC), in partnership with School Health Clinics of Santa Clara County and Pacific Clinics, provides a comprehensive array of services to stabilize people in crisis, and provide the appropriate treatment for those with substance use and mental health disorders. Our services include the following:
•Crisis mental health services
•Screening, assessment and diagnosis, including risk assessment
•Patient-centered treatment planning
•Outpatient mental health and substance use services
•Primary care screening and monitoring of key health indicators/health risk
•Targeted case management
•Psychiatric rehabilitation services
•Peer support and family supports
•Intensive, community-based mental health care for members of the armed forces and veterans
We believe that everyone deserves high quality healthcare and these services are offered on a sliding scale, regardless of the ability to pay. This work is funded by a two-year grant from the Substance Abuse and Mental Health Services Administration (SAMHSA), a federal agency with a mission to reduce the impact of substance use and mental illness in the United States.
