Educational Programs

Educational Programs provide students with the skills they need to live successful lives. Our robust programs include early childhood development, classroom consultations in partnership with school districts, adult continuing education programs and parent workshops.

Support Services

Support Services offer various programs to address social determinants of health, including housing and employment coaching and placement, among other critically needed services.

San Jose Spotlight recently reported on the “Trusted Response Urgent Support Team”, or TRUST, which is a partnership between three behavioral health agencies in Santa Clara County: Pacific Clinics, Momentum for Health and HomeFirst Services.

Following community concerns about gaps in mental health services, Councilmembers unanimously voted to explore adding another TRUST mobile field team in this year’s budget to help those in need of behavioral health support with low-risk de-escalation assistance, referrals to other community agencies for support, safety planning and stabilization services.

The TRUST team is comprised of licensed professionals and individuals with lived experience, which often results in a deeper connection to the challenges that individuals in need may be facing. They respond to each call with a listening ear, meeting community members where they’re at, at that moment in time, and strive to bring a calming presence to all situations, recognizing that a lack of sensory overstimulation (such as lights and sirens) can contribute positively towards de-escalating stressful situations.

TRUST’s call center and field response teams are available 24/7 to answer calls for help and seek to provide behavioral health and basic medical assessment services through phone or in-person support.
