Support Services offer various programs to address social determinants of health, including housing and employment coaching and placement, among other critically needed services.
A safe home is key on the path to recovery from trauma and other behavioral health challenges. Pacific Clinics assists more than 300 eligible clients who are unhoused with services and support on their way to recovery. Our housing options and supportive services help people develop social skills and receive assistance with daily activities.
Clients will work with a housing specialist who assists with creating a plan to meet goals, such as completing housing voucher applications or working with landlords. Based on the plan, clients might work with a team of people, including an employment specialist or our community partners.
We maintain locations throughout California that provide a home for men, women and children who are unhoused. Additionally, through partnerships, we have established relationships with motels or other multi-family residencies to provide short-term housing to those in urgent need. Additionally, we provide workshops for our resident clients such as budget management, social living skills, cooking and hygiene.
We provide linkages to permanent housing throughout California for transitional-age youth and adults ages 18 and older. We work with partners to find homes that meet client needs. We will also assign a case manager to help with building skills.
Our Transitional Housing Programs (THP) provide support to those of transitional age and/or transitioning out of foster care or juvenile probation and assist in their journey to independence by providing life skills coaching, basic necessities and more, all in a safe, housed environment. Each client is served through one-on-one training in shared housing units leased by our organization while having the support of supervision, mentoring and a safety net while they work toward academic or employment goals.
