What is health navigation?

Health Navigation is a culturally responsive, promising practice intervention designed to provide the services, skills and confidence, which consumers with serious mental illness need, to self-manage their healthcare.

We achieve this by connecting you with a certified health navigator who will teach you the skills to empower you to care for your medical and dental needs independently.

What can health navigators do for you?

• Guide you through the healthcare system and provide personal and individualized support.
• Help you schedule follow up or specialist appointments.
• Work with you to set your health and wellness goals.
• Assist you with scheduling and keeping appointments with medical, dental and eyecare providers.
• Accompany you to appointments and help you ask question to advocate for yourself.
• Teach you to create and keep a schedule.

Do I need a health navigator?

• Has it been a long time since your last doctor’s appointment?
• When you are sick, do you have a regular doctor or are you having trouble finding a doctor?
• Do you have a primary care doctor, but don’t feel satisfied or comfortable with the care you are receiving?
• Have you ever visited the emergency room for medical care but felt frustrated by the long wait times?
• Have you ever felt judged by your doctor’s office because of your mental health diagnosis?
• After your appointment, do you have trouble remembering or understanding your doctor’s instructions?
• Do you get confused by knowing which doctor to call for what’s bothering you?
• Did you have a not-so-good experience that makes you want to avoid going to a doctor
