We offer comprehensive care management for those in need. Pacific Clinics advocates and helps navigate the health care system for people who qualify for Medi-Cal’s Enhanced Care Management program and provides helpful resources.
Are you a Medi-Cal member and dealing with mental health challenges? You can better manage your health care through California’s Enhanced Care Management program. Too many people with chronic mental health challenges don’t have a primary care doctor who treats them. This program lets clients receive the care they need with the help of dedicated case managers.

Qualifying members who join the Enhanced Care Management program can transform their lives. Work with a dedicated team of professionals who work with you and on your behalf. You will receive proper medical care that addresses chronic or high-risk physical and mental health conditions. You can also access community resources that address the social aspects of mental health challenges and help improve outcomes. Together, we can unify and support our community members and make a difference.
The program tackles the issue of chronic homelessness among those with mental health challenges No-cost housing is available for those who need shelter or experience homelessness as a result of their health conditions.
Members under the age of 26 have a full care team that coordinates housing and treatment plans. These team members also serve as all-around advocates to provide support. All Medi-Cal members that qualify for this program can opt out at any time. Members won’t lose their regular health benefits by joining Enhanced Care Management.
The compassionate team at Pacific Clinics refers qualifying community members to the Enhanced Care Management program. But we don’t stop there. If you are eligible to join, one of our reputable team members will be your trusted care coordinator.
Care coordinators go above and beyond and work with your doctors, pharmacists and social workers. They keep everyone informed about your health and work to find more resources throughout your community. Our experienced care coordinators want to see every client succeed no matter what health conditions they face. We bring caring support and a judgment-free attitude to every client’s care.
The Enhanced Care Management program provides the support you need, no matter how complex your physical and mental health needs are. Pacific Clinics is happy to set you up with the resources and support from experts.
Our Member Engagement and Outreach team is always available to listen and help clients sign up for the program. Members can self-refer to this program if they need the support we provide.
