Support Services offer various programs to address social determinants of health, including housing and employment coaching and placement, among other critically needed services.
DHCS has a Technical Assistance Marketplace Initiative that allows your agency to apply for FREE training and technical assistance. Entities interested in becoming ECM or Community Support providers may “shop” for technical assistance from approved vetted vendors such as Pacific Clinics. PATH pays for technical assistance projects and the approved services are free for entities that successfully apply and get approval for TA with the selected vendor partners. Work with our Pacific Clinics team and receive support with the Scope of Work, budget proposal, and goal development for customized training packages.
TA makes it possible for entities to work closely with us as your chosen TA Vendor to design and implement TA that is uniquely suited to your priorities and needs. TA goals, deliverables, and timeframes are all developed “from scratch,” and can be specific to your agency and project.
Pacific Clinics offers a wide range of TA offerings that are packaged for convenient, efficient delivery and are ready to go with minimal upfront planning. Off-the-shelf TA projects have clear, pre-established objectives and defined timeframes for easy consideration and implementation yet can be customized to meet your priorities and needs.
This initiative provides technical assistance resources to providers, community-based organizations (CBOs), counties, local public health jurisdictions, Tribal Partners, and others to help build their capacity to become Enhanced Care Management (ECM) and Community Supports providers, now offered through Medi-Cal managed care plans.
To access TA resources on the TA Marketplace, organizations must submit a TA Recipient Eligibility Application, which DHCS will use to verify their eligibility.
