Support Services offer various programs to address social determinants of health, including housing and employment coaching and placement, among other critically needed services.
Rachelle Grant is the regional executive director, integrated care/CalAIM for Pacific Clinics where she is responsible for implementing new programs statewide that expand the work and commitment to integrated care and wellness in communities.
In her current role, she oversees statewide CalAIM (California’s attempt to transform its Medi-cal system) programming for Enhanced Care Management and Community Supports, which provides non-clinical services that address social drivers of health to the most vulnerable communities, including unhoused families, people experiencing frequent hospitalizations and children who have experienced significant trauma.
Having worked at Pacific Clinics since 2011, Grant has held various roles, including facilitator/clinician and clinical program manager. Among many of her accomplishments, she supported the launch of one of the first-ever Certified Community Behavioral Health Clinics (CCBHC), a clinic that provides integrated mental health and substance use services, along with physical health care, in the Bay Area and Los Angeles / South Coast regions.
Grant is a Licensed Marriage and Family Therapist (LMFT) and has a master’s degree in counseling psychology from the University of Oregon, and a bachelor’s degree in psychology from the University of California, Santa Cruz.
The recent wildfires have deeply tested our resilience as the devastating fires in Southern California continue to impact countless lives. Tragically, 40 of our employees and clients have lost their homes, and many more have been evacuated as the emergency unfolds. Our teams continue to assist clients and families during this time.
