Support Services

Support Services offer various programs to address social determinants of health, including housing and employment coaching and placement, among other critically needed services.

Pacific Clinics TRUST Program Manager Elyse Malcolm was the guest on a recent episode of Santa Clara County Supervisor Susan Ellenberg’s podcast, “And Just One More Thing,” where they discussed the TRUST program and its impact on the community.

The Trusted Response Urgent Support Team, or TRUST, is a community service provided by Pacific Clinics and Momentum for Health that helps Santa Clara County residents during urgent mental health or substance use situations.

“If you’re out in the community and see somebody struggling — they’re not committing a crime, they’re not hurting anybody, but they definitely need help — and you would like to be that neighbor that offers that help, TRUST is a wonderful place to start,” says Malcolm. “[This is because] we can go out and assess what is going on and either provide that support, or we can then activate the next team, or collaborate with another supportive service to get that person the help they deserve.”
