Educational Programs
Educational Programs provide students with the skills they need to live successful lives. Our robust programs include early childhood development, classroom consultations in partnership with school districts, adult continuing education programs and parent workshops.
Support Services

Support Services offer various programs to address social determinants of health, including housing and employment coaching and placement, among other critically needed services.

The Crossroads Transitional Housing Program (Crossroads THP) serves young adults, aged 18-24, who are currently homeless or in danger of becoming homeless in our Central Region.

What is Crossroads Transitional Housing?

Crossroads THP started at Uplift Family Services in March 2007 with the goal of helping transitional-aged youth develop the necessary skills for independent living and move into stable, permanent housing. Stable housing comes in many forms, but generally the program aims to help the youth get a place of their own or with a roommate, transition to another housing program such as low-income housing, or reunite with family in a healthy environment.

Other goals include saving money to aid in the transition to independent living, purchasing a car  for those who are financially stable enough, finding and maintaining employment to gain valuable work experience, improving on their wellness and recovery journey, and completing high school, earning their GED, or furthering their education in college or with vocational training.

How Does Crossroads Transitional Housing Work?

Young adults are referred to Uplift Family Services from one of five clinical partners in the Tulare area. Once enrolled in Crossroads Transitional Housing, the young adult is moved into a two-bedroom apartment that they share with a roommate. Each roommate has their own bedroom with shared living spaces.

The young adults in the program are provided with a structured living environment including program rules, weekly case management, therapy, and additional services as needed. Case managers, Uplift Family Services staff, and other caregivers work as a team alongside program enrollees to create a two-year wellness journey and guide the young adults through it. A typical wellness journey includes milestones such as:

  • Obtaining and maintaining employment
  • School enrollment for either GED or post-secondary education and training
  • Medication recommendations and management
  • Promoting sober living
  • Support groups
  • Providing other resources as needed

Uplift Family Services also helps young adults with their transition to independent living through everyday skills such as:

  • Money management and budgeting
  • Apartment maintenance
  • Managing food stamps
  • Saving money, paying bills, and building credit
  • Doing household chores like laundry and cooking
  • Establishing and building upon healthy relationships with family and friends
  • Accessing additional resources outside of Uplift Family Services
  • Volunteering in the community

Enrollees are also encouraged to participate in wellness events and recovery activities organized by Tulare County and other partner programs.

Over the course of its 14 years, the Crossroads Transitional Housing Program at Uplift Family Services has served 260 young adults, with 241 being successfully discharged from the program.  During the 2019-2020 fiscal year, 9 young adults were successfully discharged, 8 young adults were admitted, and 35 young adults were served in total.
